Aims: To describe a Multiple Sclerosis Manifested by Paralysis of Cranial VI Pair with Diplopia.
 Presentation Case: A.P.R. female patient, 31 years old, has presented diplopia for 04 days. She denied too many symptoms and comorbidities. No eye trauma previous surgery.
 Discussion: (MS) is chronic, immune-mediated demyelinating disease the central nervous system that mainly affects young adults. About 85% individuals MS start clinical picture in form relapse, less can open progressive neurological deficits, although occasional relapses occur during course disease. Eye changes are frequent often first manifestation.
 Results: rare comorbidity there no exact concrete epidemiological studies so far. Studies about alterations also scarce Latin American countries.
 Ocular involvement may be sign MS. Although an uncommon cause cranial nerve palsies, its frequency increases individuals, predominance abducens palsy, as occurred patient present report, emphasizing importance knowing profile this disease.
 Conclusion: findings include optic neuritis, retinitis, peripheral vasculitis, ocular motility abnormalities manifest or nystagmus, these manifestations should recognized ophthalmologists.
